Social Care Practitioner

Excel Healthcare Recruitment are currently recruiting for a Social Care Practitioner to join a residential service supporting vulnerable individuals with a range of social, healthcare, and integration needs. The successful candidate will support resident wellbeing, assist with access to essential services, and help promote positive integration within the local community.

Job Title: Social Care Practitioner
Location: Limerick / Clare
Contract: Full-Time, Permanent

Main Duties:

  • Support the wellbeing and welfare of residents.
  • Develop and implement Integration Plans.
  • Identify and support vulnerable residents and those with additional needs.
  • Build links with local services including healthcare providers, schools, charities, community groups, and voluntary organisations.
  • Establish relationships with national organisations such as the Irish Red Cross, Irish Refugee Council, Women’s Aid, and the Migrants Rights Centre Ireland.
  • Respond to resident queries, concerns, and complaints in a compassionate and professional manner.
  • Signpost and refer residents to relevant support services.
  • Maintain positive working relationships with residents, colleagues, and external agencies.
  • Participate in ongoing training and development.
  • Ensure all policies and procedures are followed and maintained.
  • Carry out any other duties as required by the Line Manager.

Essential Requirements:

  • QQI Level 7 qualification in Social Care, Applied Social Studies, or equivalent.
  • Previous experience in a social care, residential, or community-based setting.
  • Excellent communication and interpersonal skills.
  • Strong understanding of person-centred care and safeguarding practices.
  • Ability to work independently and as part of a team.
  • Full driving licence desirable.
