Job Title: Senior Occupational Therapist – Adult Primary Care

Contract Type: Full-time, 35 hours per week, Monday-Friday
Contract Duration: 1 Year Specified Purpose Contract (with strong potential for extension)
Salary: Aligned with HSE Senior Grade Occupational Therapist Pay Scales
Excel Recruitment are recruiting a Senior Occupational Therapist on behalf of a well-established Primary Care team based in New Ross, Co. Wexford. This is an outstanding opportunity for an adult trained Occupational Therapist to lead and deliver an adult focused Occupational Therapy service within a community setting.
You will join a well-established team of Healthcare professionals, where regular supervision, peer support, and innovation are at the core of service delivery.
Job Purpose:
The purpose of this role is to deliver and lead high-quality, evidence-based Occupational Therapy interventions for adults with a variety of physical, cognitive, and functional needs. Working within the Primary Care Team, the Senior OT will play a critical role in enabling individuals to live independently, manage daily tasks, and engage fully in their communities.
Clinical Responsibilities
- Provide comprehensive occupational therapy assessment, intervention, and review for adult clients, with a particular focus on older adults and those living with frailty.
- Carry out home visits to assess functional ability, environmental safety, and independence in daily living, recommending and implementing appropriate interventions and equipment.
- Deliver interventions aimed at maximising independence, safety, and quality of life, including functional rehabilitation, falls prevention, cognitive and environmental assessments.
- Work with clients in outpatient clinics, providing tailored rehabilitation programmes, education, and strategies to support participation in everyday activities.
- Develop and implement individualised care plans using evidence-based practice and clinical reasoning, ensuring that interventions are goal-directed and outcome-focused.
- Monitor and evaluate intervention outcomes, adapting care plans as necessary in collaboration with service users, families, and the wider multidisciplinary team.
- Provide guidance and education to clients, families, and carers regarding safe and effective ways to support independence at home.
- Work as an integral member of the Primary Care Team, collaborating closely with Physiotherapists, Public Health Nurses, GPs, Social Workers, Speech and Language Therapists, and other allied health professionals.
- Liaise with community services, voluntary agencies, hospitals, and other stakeholders to support coordinated and seamless patient care.
- Participate in frailty pathway initiatives, integrated care programmes, and joint assessments with other disciplines when required.
- Advocate for clients and contribute to discharge planning and community reintegration where appropriate.
- Provide clinical leadership and support to junior staff, students, and therapy assistants, including supervision, mentoring, and delegation of appropriate tasks.
Requirements:
- CORU-registered Occupational Therapist with current professional registration.
- Recognised qualification in Occupational Therapy (Degree or Higher Diploma) approved by CORU.
- Minimum 3 years post-qualification experience, ideally with at least 1-2 years in adult primary care or community-based services.
- Experience working with older adults, frailty, rehabilitation, and functional assessments.
- Demonstrated experience in home visits, outpatient clinics, and community-based assessments.
- Proven ability to manage complex caseloads independently.
- Expertise in assessment and intervention for frailty, falls prevention, mobility issues, cognitive impairment, and activities of daily living.
- Strong knowledge of equipment prescription, environmental adaptations, and assistive technology to support independence.
- Ability to develop, implement, and evaluate care plans using evidence-based practice.
- Experience in multidisciplinary team working and interagency collaboration.
