Contract: Full-time, 3-6 months with strong potential for extension
Working Hours: Monday-Friday, 35-hour week

Excel Recruitment is proud to partner with a Community Primary Care team in Tallaght to recruit a Professionally Qualified Social Worker for a full-time contract position.

This is an excellent opportunity to contribute meaningfully to the health and wellbeing of adults and families within the Tallaght community, working as part of a supportive multidisciplinary team.

Job Purpose:

The Social Worker will deliver person centred support to adults accessing community health services, with a focus on psychosocial assessment, care planning, advocacy, and safeguarding. This role plays a key part in supporting individuals and families facing health-related challenges, social isolation, bereavement, mental health concerns, and more.

Key Responsibilities:

  • Conduct psychosocial assessments and develop individualised support plans.
  • Provide emotional and practical support to individuals and families in the community.
  • Assist service users in accessing relevant health, social, and community-based services.
  • Collaborate with GPs, Public Health Nurses, Occupational Therapists, Physiotherapists, and other primary care professionals to ensure holistic care.
  • Advocate for clients’ rights and support them through complex social or health-related situations.
  • Participate in safeguarding assessments and procedures in line with HSE and national policies.
  • Maintain professional and timely case notes and reports in accordance with best practice and data protection standards.
  • Attend multidisciplinary team meetings and contribute to case reviews, service planning, and continuous improvement initiatives.
  • Engage in supervision and ongoing professional development activities.

Essential Criteria:

  • A recognised professional qualification in Social Work.
  • CORU registration as a Social Worker (essential).
  • Experience working in a primary care, hospital, or community setting.
  • Strong understanding of social determinants of health and safeguarding frameworks.
  • Excellent communication, assessment, and organisational skills.
  • Ability to work both independently and as part of a collaborative multidisciplinary team.
  • Full clean driving licence and access to own transport.
  • Valid Garda vetting and overseas police clearance (if applicable).
