Duties of the Clinical Pharmacist:
- Carry out medicines reconciliation on admission, transfer, and discharge
- Review and optimise medication regimens with a focus on polypharmacy and deprescribing
- Provide clinical advice on dosing, interactions, and monitoring of high-risk medicines including anticoagulants, insulin, antibiotics, and opioids
- Support patient and family education on safe use of medicines on discharge
- Participate in ward rounds and discharge planning meetings with the wider multidisciplinary team
- Contribute to medication safety audits, antimicrobial stewardship, and quality improvement initiatives
- Provide training and support to nursing and care staff on safe medicines administration
Clinical Pharmacist must:
- Hold a Degree in Pharmacy (MPharm or equivalent)
- Be registered with the Pharmaceutical Society of Ireland (PSI)
- Have 1-2 years’ post-qualification experience in a hospital, primary care, or transitional care setting
- Experience in geriatric medicine, rehabilitation, or polypharmacy management is desirable
