Job Title: Senior Occupational Therapist – Older Adults & Primary Care

Excel Recruitment is recruiting an experienced Senior Occupational Therapist on behalf of a well established Adult Primary Care Service in Dún Laoghaire. The successful candidate will deliver a responsive, person-centred service across clinic and community settings, with a strong focus on home assessment, lone working, equipment prescription and supply, environmental recommendations and complex case management.

Overview:

The Senior Occupational Therapist will assess and support older adults and other adult service users presenting with physical, cognitive, functional and environmental needs. The caseload may include frailty, reduced mobility, falls risk, neurological conditions, disability, cognitive change and complex home circumstances. Working independently in service users’ homes, the successful candidate will complete comprehensive functional and environmental assessments, identify risks, prescribe and arrange the supply of appropriate equipment, recommend adaptations and coordinate interventions with families, carers and the wider multidisciplinary team. This role will suit a confident community practitioner who can prioritise effectively, make sound clinical decisions and work safely while lone working.

Clinical Responsibilities:

• Manage a diverse adult community caseload, with a particular focus on older adults, frailty and complex functional needs.
• Complete comprehensive home-based assessments of functional ability, mobility, cognition, personal care, transfers, falls risk and environmental safety.
• Work independently during community visits, applying lone-working procedures, dynamic risk assessment and appropriate escalation.
• Assess for, prescribe and coordinate the supply of equipment, including mobility, transfer, bathing, toileting, seating and pressure-care solutions.
• Recommend minor and major home adaptations and prepare supporting clinical reports where appropriate.
• Develop, implement and review practical, goal-focused intervention plans that promote independence, safety and participation.
• Support service users experiencing frailty, neurological conditions, disability, cognitive change, reduced mobility or difficulty completing daily activities.
• Provide clear advice, education and practical guidance to service users, families and carers.
• Collaborate with GPs, Public Health Nurses, Physiotherapists, Social Workers, Speech and Language Therapists, hospitals, community services and equipment suppliers
• Provide clinical leadership, supervision and support to junior Occupational Therapists, therapy assistants and students as required.

Requirements:

• CORU registration as an Occupational Therapist and a recognised Occupational Therapy qualification.
• Experience working with older adults and/or in primary care, including home visits, lone working, and equipment prescription and supply.
• Demonstrated competence in home visits, autonomous practice and safe lone working.
• Strong practical knowledge of equipment assessment, prescription, ordering, supply pathways and follow-up review.
• Experience assessing functional performance, environmental risk, seating, transfers and home adaptation needs.
• A full driving licence and access to a car, as regular travel and home visits are integral to the role.
• Excellent clinical reasoning, caseload management, communication and documentation skills.
• A practical, organised and person-centred approach, with the confidence to make decisions independently and seek support appropriately.
