Excel Recruitment are currently recruiting a CORU-registered Social Worker to join a well established Adult Primary Care Team based at Millbrook Lawns Health Centre in Tallaght, Dublin 24.

There may also be a requirement or opportunity to work from Kilnamanagh Tymon Primary Care Centre, also in Dublin 24. The two locations are approximately a five-minute drive apart.

This is a rewarding role within a multidisciplinary team delivering person-centred care to adults with a wide range of health, social, and psychosocial needs.

You will work with adults across the lifespan, supporting individuals experiencing chronic illness, disability, mental health challenges, social isolation, safeguarding concerns, and difficulties accessing community supports.

This role is ideal for a Social Worker with a strong interest in community-based practice, empowerment, advocacy, and strengths-based interventions.

Key Responsibilities:

  • Carry out comprehensive psychosocial assessments of adults presenting with a range of health and social care needs.
  • Provide emotional support, counselling, advocacy, and practical assistance to individuals and families.
  • Support service users to access appropriate health, housing, welfare, and community supports.
  • Work collaboratively within a multidisciplinary team including Occupational Therapists, Physiotherapists, Nurses, Psychologists, and other professionals.
  • Contribute to care planning and review processes, ensuring interventions are person centred and goal focused.
  • Participate in adult safeguarding work in line with HSE policies and national legislation.
  • Build and maintain effective links with voluntary agencies, community organisations, and statutory services.
  • Maintain accurate, high-quality records, reports, and documentation in line with professional and HSE standards.
  • Promote dignity, autonomy, inclusion, and wellbeing in all aspects of practice.

Essential Criteria:

  • Current CORU registration as a Social Worker is essential.
  • Previous professional experience in a community or primary care setting is essential.
  • A full, clean driving licence and access to a vehicle are essential for this community-based role.
  • Strong knowledge of adult safeguarding legislation, community resources, and multidisciplinary working.
  • Excellent assessment, communication, advocacy, and report-writing skills.
  • Ability to work independently while contributing effectively to a collaborative MDT.
