Excel Recruitment is delighted to partner with the HSE in recruiting a Medical Social Worker for a state-of-the-art Transitional Care Step-Down Unit in Raheen, Limerick. This permanent, full-time opportunity offers the chance to play a pivotal role in supporting patients and families during a critical stage of recovery following acute illness, surgery, or major health events.

This modern facility provides 80 single occupancy ensuite rooms in a welcoming, homely environment, designed to support rehabilitation, reablement, and safe transition from hospital to home.

Job Purpose

The Medical Social Worker will provide comprehensive psychosocial assessment, intervention, and support to patients and their families within the Transitional Care setting. Working as a key member of the multidisciplinary team, the post holder will support patients in navigating complex social, emotional, and practical challenges, facilitate discharge planning, and promote safe, timely transitions back to the community.

Key Responsibilities

  • Provide high-quality medical social work assessment and intervention to patients and families experiencing illness, trauma, or functional decline.
  • Support patients in adjusting to illness, injury, or changes in independence, offering emotional, practical, and psychosocial support.
  • Work closely with the multidisciplinary team to support holistic care planning and safe, effective discharge planning.
  • Assess social circumstances, family dynamics, housing, safeguarding concerns, and support needs.
  • Advocate on behalf of patients and families to ensure access to appropriate community supports, services, and resources.
  • Liaise with acute hospitals, community services, voluntary organisations, and statutory agencies to coordinate care and support continuity.
  • Support patients and families with complex decision-making, including capacity, consent, and care planning where required.
  • Maintain accurate, timely, and confidential records in line with professional, organisational, and legal standards.
  • Participate in multidisciplinary meetings, case conferences, and discharge planning forums.
  • Contribute to service development, audit, and quality improvement initiatives.
  • Adhere to HSE policies, procedures, and professional codes of practice at all times.

Essential Requirements:

  • Recognised qualification in Social Work.
  • CORU registration as a Social Worker (essential).
  • Experience working in a medical, healthcare, or hospital setting.
  • Strong assessment, intervention, and advocacy skills.
  • Experience working with adults and families experiencing illness, disability, or social vulnerability.
  • Excellent communication, interpersonal, and multidisciplinary working skills.
  • Ability to manage a varied and complex caseload effectively.
  • Strong understanding of safeguarding, capacity, and ethical practice.
  • Commitment to person centred, compassionate care and continuous professional development.
