Position: Qualified Social Worker – Adult Primary Care
Location: Portarlington / Mountmellick, Co. Laois
Contract: 5-Month Agency Contract

Excel recruitment are recruiting a Qualified Social Worker to join an Adult Primary Care team in the Laois area for an agency contract.

Key Responsibilities:

• Providing comprehensive social work assessments for adults with a wide range of social, emotional, medical, and environmental needs.
• Supporting service users to access appropriate community services, supports, and care pathways.
• Developing person-centred intervention plans, promoting autonomy, safety, and improved quality of life.
• Working collaboratively with the multidisciplinary Primary Care Team including nursing, physiotherapy, occupational therapy, psychology, and other allied health professionals.
• Advocating for service users, ensuring their rights, dignity, and choices are respected.
• Engaging with families, carers, and external agencies to coordinate effective support networks.
• Maintaining accurate and timely documentation, reports, and case notes in line with HSE standards.

Essential Criteria:

• CORU-registered Social Worker (essential).
• Experience working within Adult Primary Care, community services, medical social work, or a related area.
• Strong knowledge of community resources, social supports, and HSE care pathways for adults.
• Excellent communication, assessment, and advocacy skills.
• Ability to manage a varied caseload and work both independently and as part of a multidisciplinary team.
• Full clean driving licence and access to transport (essential).
