Occupational Therapist (Adult Primarycare)
Job Title: Staff Grade Occupational Therapist (Adult)
Location: New Ross, Co. Wexford
Contract Type: Full-time Agency Contract
Working Hours: 35 hours per week
Salary: Aligned with HSE pay scales for the appropriate grade.
Excel Recruitment is proud to partner with a well-established Primary Care team in New Ross to recruit a dedicated Staff Grade Occupational Therapist.
This is an excellent opportunity for an Occupational Therapist with an interest in adult rehabilitation, frailty, and community-based care to join a supportive and progressive multidisciplinary team.
The Occupational Therapist:
- Will provide comprehensive occupational therapy assessments and interventions for adults, with a primary focus on frail elderly clients.
- Deliver community and home-based rehabilitation (approx. 20% home visits), supporting falls prevention, cognitive rehabilitation, self-care reablement, and community living skills.
- Provide clinic-based interventions, including: lifestyle redesign groups, assistive technology training, complex seating assessments, upper limb rehabilitation.
Clinical Duties:
- Conduct comprehensive occupational therapy assessments for adults with a range of conditions, including physical, cognitive, and psychosocial impairments.
- Assess clients in home environments to evaluate functional ability, independence, and safety, including mobility, self-care, and daily living activities.
- Conduct assessments in outpatient clinics to identify rehabilitation needs, functional limitations, and areas for intervention.
- Utilise standardised assessment tools and clinical reasoning to inform interventions and care plans.
- Develop and implement individualised intervention plans to enhance independence, functional ability, and quality of life.
- Provide therapeutic input addressing mobility, self-care, cognitive, sensory, and psychosocial needs.
- Prescribe and provide assistive devices, adaptive equipment, and environmental modifications to promote safe and independent living.
- Deliver rehabilitation programmes aimed at falls prevention, frailty management, and functional recovery following illness or injury.
- Provide education and training to clients, families, and carers to support safe and effective daily living at home.
- Conduct home visits to assess clients’ living environments, recommend adaptations, and provide interventions to maximise independence.
- Evaluate risks and implement strategies to reduce hazards and promote safety in the home.
- Support clients in community participation, including mobility, social engagement, and access to community resources.
- Work as part of a multidisciplinary Primary Care team, collaborating with Physiotherapists, Nurses, Social Workers, Speech and Language Therapists, and GPs.
- Liaise with external services, voluntary organisations, and hospital teams to ensure continuity of care and coordinated service delivery.
- Attend case conferences and team meetings to review and plan client care.
- Maintain accurate and timely clinical records, reports, and care plans in line with HSE and CORU standards.
- Engage in continuous professional development (CPD) to maintain up-to-date knowledge and clinical skills.
Requirements:
- Be registered on the Occupational Therapists Register maintained by the Occupational Therapists Registration Board at CORU.
- Experience working with adult and/or older adult populations (Essential)
- Strong clinical reasoning and problem-solving skills.
- Excellent communication and teamwork skills
- A team player with excellent communication skills and a passion for working with children.
- A full driver’s license and access to your own vehicle.
- Current residency in Ireland with the legal right to work without restrictions.
