Duties of the Clinical Pharmacist:

• Carry out medicines reconciliation on admission, transfer, and discharge
• Review and optimise medication regimens with a focus on polypharmacy and deprescribing
• Provide clinical advice on dosing, interactions, and monitoring of high-risk medicines including anticoagulants, insulin, antibiotics, and opioids
• Support patient and family education on safe use of medicines on discharge
• Participate in ward rounds and discharge planning meetings with the wider multidisciplinary team
• Contribute to medication safety audits, antimicrobial stewardship, and quality improvement initiatives
• Provide training and support to nursing and care staff on safe medicines administration

Clinical Pharmacist must:

• Hold a Degree in Pharmacy (MPharm or equivalent)
• Be registered with the Pharmaceutical Society of Ireland (PSI)
• Have 1-2 years’ post-qualification experience in a hospital, primary care, or transitional care setting
• Experience in geriatric medicine, rehabilitation, or polypharmacy management is desirable
